Housing Transition
Services provided directly (in person or remotely) or indirectly cater to diverse housing needs, from developing transition plans to securing benefits and navigating the housing search process. Our dedicated support ensures every step, whether budgeting, landlord negotiations, or cultural considerations, is expertly guided, fostering safe and stable living arrangements for all.
Housing Consultation
Enrolled housing consultants assist individuals without waiver or targeted case management to develop a Housing Focused Person-Centered Plan, ensuring eligibility for Housing Stabilization Services and facilitating informed decisions in selecting housing transition or sustaining providers. Through comprehensive support, they identify strengths, cultural preferences, and non-housing related goals, coordinating with other service providers and updating plans annually to ensure ongoing alignment with housing needs.
Housing Sustaining
Regularly updating and modifying housing support and crisis/safety plans is a crucial aspect of our services, ensuring ongoing stability for individuals. We also provide comprehensive support in preventing eviction risks, promoting cultural understanding with landlords, and arranging necessary accommodations for enhanced living conditions.
MNSURE NAVIGATOR
As a MNsure navigator we assist with application filling and enrollment in coverage, including renewal support and follow-up. Our services, which encompass reporting changes and providing assistance, are offered at no cost.

Who We Serve
- Housing Stabilization is for adults with a mental illness who reside in Stearns County and surrounding areas.
- Individuals must have Medical Assistance to receive Housing Stabilization Services.
- Individuals must have a CADI or BI Medical Assistance Waiver to receive Home and Community Services
