Housing Consultation

Our enrolled housing consultants assist individuals without waiver or targeted case management to develop a Housing Focused Person-Centered Plan. This plan ensures eligibility for Housing Stabilization Services and facilitates informed decision-making in selecting housing transition or sustaining providers.

MNsure Navigator

As MNsure navigators, we offer free assistance with healthcare coverage applications and renewals. Our services include reporting changes and providing ongoing support to ensure continuous coverage.

Moving Home Minnesota

Stable Home Service proudly offers the Moving Home Minnesota (MHM) program, designed to help individuals transition smoothly from institutional settings to community-based living. Our expert team provides personalized transition planning, comprehensive support, and seamless integration into the community.
